What should a clinic abroad send to the treating rheumatologist?

I am writing on behalf of MedTraveling. Our project compares clinics abroad, and I would like to ask about the information a clinic should provide when someone has a rheumatic condition. The directory is at https://medtraveling.com/clinics. A listing alone cannot tell us whether a particular clinic is suitable for someone. What would you want the clinic to send to the treating rheumatologist before any booking? Would a named contact for follow-up and a written plan for sharing records be useful, or is there another detail that often gets missed? I am asking about the process, not for advice on changing medication.
